Their services include information, referral, and outreach about the many services and benefits available to older adults, answering questions about government benefits and community resources such as Social Security, Medicare, Medicaid, and the Benefit Access Program, advocating with other agencies on a client’s behalf, and assisting with forms and applications for benefits. Their Case Management Services assess the need for care, develop an overall plan, and follow up to ensure clients are receiving the services they need, including home delivered meals and in-home services.
Their staff can also help seniors and caregivers find housing options including affordable housing, assisted or supportive living facilities, and nursing homes across Kane, Kendall, and McHenry Counties. They also have several programs to help isolated seniors, matching them with carefully screened friendly visitors or companions through their Friendly Visiting Program.

Located at 1401 IL-176 in Crystal Lake, it is one of the more accessible social options for seniors in the Lake in the Hills area who are living independently and want to stay active and connected.
Senior Care Volunteer Network
The Senior Care Volunteer Network supports senior neighbors who live independently in their own homes or with loved ones, connecting community volunteers with seniors to help. They also maintain a supply of durable medical equipment, which can be a lifesaver for families navigating a recovery or discharge from the hospital.
